In print layout, my header and footer are visible, formatted correctly, looks great. I've looked through the forums and found a few cases that may be identical to mine, but none of them actually solved the problem for the original poster (even though many thought they did).

In the drop-down list that appears in the Footnotes area, choose Footnote Separator. Header/Footer Disappears when attempting to edit.
